We strive to make your visit with us as easy and straightforward as possible. Please remember to bring the following to your upcoming eye care visit:

A valid ID and insurance card (if available)

Your previous eyewear and/or contact lens prescription

A list of your medication (if applicable)
Please arrive 15 min early to allow for ample time to complete necessary in-office paperwork. For your convenience, you can complete the online check-in via the Patient Portal.
